Once we receive your consultation payment, then you will receive a link to complete your new patient registration
NOTE: your SSN or driver's license is not required. Feel free to omit.
SCHEDULE
A Smile Team member will reach out to you within 24 hours via email or phone once we've received your online patient registration forms to help you select a day and time for your formal in-person consultation
MEETING
Get ready for your in-person consult:

Remove and leave all of your facial piercings and jewelry at home (earrings, necklaces, etc.)

Minimal make up
-
Plan for an up to 2-hour meeting
Practice COVID-19 Safety & Prevention Policy

Practice Communicative Diseases Liability Waiver must be signed in your online registration in order for you to proceed with your visit

Mask upon entering to our practice is optional

Questionnaire may be conducted at your appointment and/or on your appointment confirmation call

Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:

Fever or chills
-
Cough
-
Shortness of breath or difficulty breathing

Fatigue
-
Muscle or body aches
-
Headache
-
New loss of taste or smell
-
Sore throat
-
Congestion or runny nose
-
Nausea or vomiting
-
Diarrhea

Temperature may be taken upon arrival (forehead - touchless) and if taken, it must be less than 100.4 F (if you have 2 out 3 readings above 100.4 F, then you will be considered to have a fever)

If you were presumed or tested COVID-19 positive through a rapid test or contact tracing, then we will need to reschedule your appointment. In order for us to reschedule you, we will need a confirmed negative test date within the last 3 days (negative home tests are not accepted)

Your appointment will be cancelled and you will be asked to leave the practice if we witness any severe "cold" symptoms listed above

If you've been fully vaccinated and/or boosted, please bring your vaccination records so we can make a copy for our records
